When starting Speech Dispatcher with either LANGUAGE or LANG variables set, if DefaultLanguage was not set in speechd.conf, the value from these 2 variables should be used for the default language.

E.g. if speechd.conf contains the line about DefaultLanguage commented out, just like in the speechd.conf shipped by Speech Dispatcher:
# DefaultLanguage "en"
and LANGUAGE is set to cs_CZ:en_US:en (or LANG set to cz_CZ.UTF-8), then "spd-say hello" should speak in Czech.

I think this is more about the clients. As you correctly note, spd-say should ask for Czech when the environment is set to Czech.
